Parents- 

All students planning to participate in Rock Creek USD 323 Athletics grades 9-12 must have the following FOUR forms completed, signed, and turned into the appropriate Activities Office for RC High School.  This must be done prior to students being allowed to participate in any practices for their activity.

Please note that all KSHSAA Sports Physicals must be dated following May 1, 2024 to be valid.  Students will not be able to use Sports Physicals or paperwork from previous years to meet these requirements.
